Hi MM. In 1955 the rules were much stricter than in later years.
In 1955 in Newcastle anyway, if Jack Pye had hit The Ghoul outside of the ropes he may have been disqualified. Certainly he would have been warned and no count would have taken place.
As to all of your questions the answer is that the promoter would have decided before hand what would take place.
