We've had discussion before of Paul Lincoln returning to the UK and possibly assuming the role. Possibly for Joint, but we've also been told of him moving up north and working for his friend Don Robinson. I've never paid too much attention to the timespan of these comments. What makes you think this was the original Anglo Italian?
Gori Ed Mangotich was Dr Death on the independents around this time. That photo has intrigued me too, doesn't really sell it to the fans.
The DM wouldn't use copycats argument wouldn't hold any water by 1977.
Sorry, I have no idea, however if I might pick up from the text, what business was Clayton Thompson concentrating on?
There was several Dr. Death's . Another one was John " Jock " Ruddock , in New Zealand .

Except that this is dated 1977 and Paul Lincoln went back to Australia for 11 years, in 1975, coming back to the UK in 1986.
Sure, it's a similar mask, but why no black leotard, to give it the Doctor Death image that Lincoln had successfully built over many years?
Not saying 100% it's not Paul Lincoln, but it seems unlikely, given the time scale.
Dale Martin didn’t promote copycats
if you wanted to use a copycat why use a pot-bellied middle-aged man?
the distinctive mask type.
it looks like Paul Lincoln.
if you want to use a copycat, use him full-scale rather than just on a few bills in the Hampshire area.
No doubt this was Paul Lincoln.
Could be Buddy Ward. Very similar look (although with a mask on of course).
Everybody turned into Dr Death one day.
